Mountain Valley Hospice is seeking nominations from the community to help identify this year’s candidates for the Gregory R. Hoye Award. This award is an honor that has been bestowed annually by the Hoye family and Mountain Valley Hospice. The award is bestowed upon an individual who has provided distinguished care to those with a limited life expectancy. Nominations received will be reviewed and the individual will be recognized at the Annual Hospice Dinner on Thursday, November 6th at The Center at Lexington in Johnstown.
With an increasing awareness of end of life issues, Mountain Valley Hospice is pleased to continue our recognition of excellent care-giving to patients and their families. The hospice concept promotes patient self-determination while planning interventions to meet the physical, emotional and spiritual needs of the patient and family. Caregivers who have demonstrated competence and compassion in this area of care giving, warrant recognition and praise.
Anyone who would like to submit a nomination is asked to submit the attached form or contact Mountain Valley Hospice at 725-4545.
